Civil Registry

As mandated by the Local Government Code, the Local Civil Registry is tasked to accept registrable documents and judicial decrees affecting the civil status of persons; file and preserve in a secure place the books, transcribe upon receipt all registrable documents; transmit to the Office of the Civil Registrar-General duplicate copies of registered documents; issue certified transcripts or copies of any certificate or registered documents; receive applications for the issuance of a marriage license and coordinate with the National Statistics Office in conducting educational campaigns for vital registration and assist in the preparation of demographic (Section 479, Article IX of Republic Act 7160).

Requirements for the Issuance of Certificate of Live Birth
For on-time registration:
Marriage contract of Parents
Signature of attendant at birth
Barangay Midwife’s certification of the circumstances of the birth

Delayed Registration
National Statistics Office Certificate of Birth (1945-1989)
Baptismal Certificate of the Child
Marriage Contract of Parents
Signature of attendant at birth (if still living)
Affidavit of two disinterested persons
Recent Community Tax Certificate of the informant
At least three documentary evidences/public documents showing the correct full name, date of birth and place of birth of the child

If parents are not married:
Signature of attendant at birth
Personal appearance of the father at the Municipal Civil Registrar (if the father is willing to admin paternity of the child) and his recent Community Tax Certificate)

Steps for the On-Time Registration of Child for Married/Unmarried Parents
1. Fill out and submit application and requirements for birth registration and provide need information during the interview. The Registration Officer/Clerk prepares a Certificate of Live Birth based on the information supplied. Transaction Time: 15 minutes
2. Review the document and affix signature on the space provided. The Registration Officer/Clerk encodes the COLDB. Transaction Time: 5 minutes
3. Wait the Registration Officer to record the documents. The Registration Officer/Clerk will assign a registry number to the COLB. Transaction Time: 10 minutes.
4. Claim the registered documents. The Registration Officer/Clerk issues the COLB and records the issuance.

Delayed Registration of Child for Married/Unmarried Parents
1. Submit application of birth registration and other required documents and provide needed information during the interview. The Registration Officer/Clerk will prepare the Certificate of Live Birth based on the information supplied. Transaction time: 15 minutes
2. Review the documents and affix signature on the space provided. The Registration Officer/Clerk encodes the COLB. Transaction time: 5 minutes
3. Inform the client date of compliance with 10 day posting period. The Registration Officer will inform the client of the date of release. Transaction time: 5 minutes
4. Claim the duly registered Certificate of Live Birth on the appointed date. The Registration Officer issues the COLB and records and issuance. Transaction Time: 5 minutes

Steps for Registering Foundling by the Finder or Charitable Institutions
Requirements:
Certificate of Foundling
Affidavit of the Finder
Certification of the Barangay Captain or Police authority regarding the report made by the finder

1. Submit the requirements for registration and provide the necessary information during the interview. The Registration Officer/Clerk will conduct the interview. Transaction Time: 30 minutes
2. Advise the foundler of the date of issuance of registration. The Registration Officer/Clerk advised for the issuance of the regisration. Transaction time: 10 minutes
3. Claim registered Certificate of Foundling. The Registration Officer/Clerk issues the certificate. Transaction Time: 5 minutes.

Steps for the Issuance of Death Certificate
Requirements:
1. If the death occured at home (on time registraion) – personal appearance of the informant who should be a relative of the deceased. Barangay Certificate on the circumstances surrounding the death.
2. If occured in the hospital (on time registration) – Death certificate prepared by the hospital
3. Sworn statement is required if registration is delayed even when death either happened at home or in the hospital

Fees:
Burial Permit – Municipal Cemetery P 110, other cemetery P 50
Transfer of Cadaver – P 100

Steps:
1. Proceed to the Municipal Registrar Office and provide the necessary information during the interview. The Registration Officer/Clerk prepares the death certificate. Transaction Time: 15 minutes
2. Review the death certificate prepared and affix your signature on the space provided. The Registration Officer/Clerk issues a copy of the accomplished but unregistered death certificate. Transaction time: 5 minutes
3. Go to the embalmer and Health Officer and have the Death Certificate signed. Transaction Time: 1 Hour
4. Pay the corresponding fees at the Municipal Treasurer’s Office. Transaction Time: 10 minutes
5. Return to the Municipal Civil Registrar with the Death Certificate and Official Receipt. Wait as the registration officer records the documents and assign a registry number. Transaction Time: 15 minutes
6. Claim the duly registered Death Certificate. Transaction time: 55 minutes

Steps for Securing a Marriage License
Requirements:
Pre-marital Counseling Certificate
Birth Certificate of Contracting Parties/Baptismal Certificate
Parental Consent (for man/woman 18-20 years old)
Parental Advised (21-24 years old)
Certificate of No Marriage (Cenomar)
Certificate of Singleness (25 and above)
Passport and legal capacity to contract marriage (for foreigner)
Personal appearance of the couple

Fees:
Pre-marital counseling – P 50
Application for Marriage License – P 100
Marriage License – P 50
Solemnization – P 100

Steps:
1.Submit all the required documents and provide the necessary information during the interview by the Registration Officer. Sign the application form. The Registration Officer/Clerk will review the documents submitted, interview both parties and instruct parties to pay fees. Transaction Time: 20 minutes
2. Pay the corresponding fees at the Municipal Treasurer’s Office. Transaction Time: 10 minutes
3. Return to the MCR and present the official receipt and entrust all the required documents for the 10 day posting period. The Registration Officer/Clerk will inform clients on the release of the marriage license. Transaction Time: 5 minutes
4. Return to the MCR Officer as instructed and claim the marriage license. Sign the logbook as sign of receipt. The Registration Officer/Clerk issues the marriage license. Transaction Time: 5 minutes

Registration of Marriage Certificate

Steps for on-time registration
1. Submit duly accomplished Marriage Certificate in Quadruplicate copies. The Registration Officer/Clerk records the documents and informs the client of the date of release. Transaction Time: 5 minutes
2. Claim the duly registered Marriage Certificate. Sign the logbook as proof of receipt. Transaction Time: 5 minutes

Steps for delayed registration
1. Submit all the required documents and provide all necessary information during interview. The Registration Officer/Clerk will review the documents for completeness and advise the client the release of certificate. Transaction time: 5 minutes
2. Claim the registered Marriage Certificate. Transaction Time: 5 minutes.

Issuance of Transcription Copies of Birth, Death and Marriage Certificates
Steps:
1. Fill out request form and submit to the receiving clerk or researcher. The Researcher will check the availability of the documents and informs the status of the requested documents. The Researcher will also inform the client the release of documents. Transaction Time: 25 minutes
2. Claim the certified true transcription copies of the documents. The Registration Officer will issue the documents. Transaction Time: 5 minutes
